Below is a link to next school year's Athletic Physical form. If your child plans to participate in activities next school year they will need to complete an athletic physical and have it turned in to Mr. Heisinger prior to being allowed to practice/compete next school year. The one-year grace period granted by the SDHSAA due to COVID-19 is no longer in effect, so all MCM athletes (7-12) will need a new physical on file for next year.
Any physicals completed on or after April 1, 2021 are good for the next school year (2021-22).
